Folic acid, an inactive form, is transformed into active forms—tetrahydrofolic acid and methyltetrahydrofolate—by the enzyme dihydrofolate reductase (DHFR). These active forms are essential for:
Common: Mild gastrointestinal upset

Adults
Initial dose: 5 mg daily for up to 4 months
Maintenance: 5 mg every 1 to 7 days, based on clinical need

In elderly patients, check Vitamin B12 levels before starting long-term folate therapy.
